Our Transportation Services aim to connect people to their community by providing safe, affordable, accessible door-to-door transportation options.
CareLink Health Transit
CareLink is a transportation service, operated in Chatham-Kent by CHAP, to ensure people are able to access health services in their community. CareLink vehicles are fully accessible. Each can accommodate up to 5 passengers at a time, including 2 passengers using wheelchairs. Destinations range from local to out-of-town locations and fares vary based on destination.
Eligible passengers are:
older adults age 60+ travelling for a medical appointment or social engagement;
adults with disabilities 18+ travelling for a medical appointment.
If a passenger requires assistance due to health or safety concerns, a family member or caregiver over age 18 must accompany them, free of charge.

To book a drive, please phone a Transportation Coordinator at 519.354.6221 ext. 242. A minimum of three business days’ notice is requested when booking drives; however, urgent drives may be accommodated as availability allows.
